A Supportive Step Between Rehab and Home

What happens when a short-term rehabilitation stay ends—but a patient isn’t quite ready to return home?

It’s a situation many families face. Progress has been made, but not quite enough to safely manage daily life independently. At that critical moment, the right support can make all the difference.

To meet this need, Friends Home in Kennett has partnered with REHAB 1 to launch a new Bridge Program—a thoughtful option designed to help individuals continue their recovery while gaining strength, confidence, and clarity about next steps

Available at Various Levels of Care

The Bridge Program offers short-term stays in both Personal Care and Long-Term Care settings, providing a safe and supportive environment for those who need a little more time.

Whether the goal is to return home or to determine a longer-term plan, residents receive individualized attention focused on achieving their highest level of function.

A key component of the program is ongoing therapy. Friends Home partners with REHAB 1 to provide physical, occupational, and speech therapy under Medicare Part B. This experienced team has been dedicated to the Friends Home community for over four years, offering one-to-one therapy sessions that build trust and support meaningful progress.

Recovery is A Team Effort

Care is highly collaborative. Weekly interdisciplinary meetings ensure that each resident’s therapy plan is carefully reviewed and adjusted as needed.

Families are included in discharge planning conversations, and when appropriate, home visits help ensure a smooth and successful transition.

Many individuals who participate in the Bridge Program are able to return home safely—and often continue their therapy on an outpatient basis with the same trusted team.

Additional Information

It’s important to note that while therapy services are typically covered under Medicare Part B, residents are responsible for a daily room and board rate.
