Our Bridge Program is for patients who have either not qualified for short-term rehabilitation or who have reached the end of their rehabilitation, but are not ready to go home just yet.
We offer the ideal, supportive next step for patients who need more time, therapy, or assistance to regain function and plan for discharge.

Bridge Program FAQs
How does the Bridge Program differ from a standard Respite Stay?
The Bridge Program is a goal-oriented, clinical transition for those who have completed hospital rehabilitation but are not yet ready to return home.
Unlike a standard Respite Stay, it focuses on intensive recovery through REHAB 1 with structured discharge planning and weekly interdisciplinary meetings.
How do I begin the admission process for a Bridge Program stay?
The process is designed to be responsive to your needs. Simply contact our Admissions Director at (610) 444-2577 ext. 202 to discuss availability.
We can also coordinate family discharge planning conferences and home visits to ensure a safe transition.
Are there any medical conditions that Friends Home cannot accommodate?
To ensure the safety and well-being of all residents in our intimate setting, we must focus on high-quality, personalized care that fits our environment.
Our facilities are not equipped to provide continuous care for residents requiring ventilator dependency.
Additionally, we cannot accommodate individuals with significant psychiatric issues that could result in them being harmful to themselves or to others.

Give-It-a-Try Stays
These 30 to 90-day, Give-It-A-Try Stays are designed to give potential residents a peek into life at Friends Home without the commitment.
Hospice Stays
For those seeking a comfortable place for end-of-life care, around-the-clock symptom management, or professional support for their family’s caregiving.
